The Handbook and templates

Handbook chapters are author drafts of generalized professional guidance. Worked examples use a fictional study and illustrative numbers; they are teaching aids, not case studies or benchmarks. Chapters show their status and review state, and a chapter marked as an outline is not finished text. Templates are provided as starting points to adapt, without any warranty of fitness for a particular study or procedure.
